1.  To create a Feedback Form on a page, first select the page from the left navigation.  Select Content and Layout to view the entire page.  Select the frame you wish to have the feedback form displayed in and click the lower case "i" icon on the toolbar for Frame Type & Info.

2.  Select Basic Frames as the Module in the first drop down.

3.  Select Feedback Form as the Type from the second drop down.

4.  Click 1save

 

This will bring you to the Frame Settings screen.  Here you will be able to change the text in the Email Subject, the From field, the To field, the CC field, Submit Confirmation, and decide what to do with the form after it's been submitted.

The Email Subject field should display text that will give you a clear indication when it arrives in your Inbox that it is from your site and from this particular form.  Especially if you are planning on having several feedback forms on your site.

 

The From drop down will allow you to have the email sent to you from the system address (system@websiteforge.com), from a Custom Email address that you type in, or from an Email Field that you create in the actual form for your customers to fill out.  This drop down can be changed after you create the form to reflect the desired result.

 

The To email field should display the email address that you want the completed form sent to.  It could be sent to the email address that you gave us upon signing up or a custom email address that you create or use.

 

The CC checkbox if checked will send the filled out form to another email address if you wish to send to more than one person.

 

The Submit Confirmation checkbox if checked will throw a warning window up after the customer clicks on the Submit button that asks them to double check their entries.

 

The Upon Successful Submit drop down allows you to Clear the Form after submission, Redirect To Page after the submission, or Show Message after submission.  Selecting Redirect to Page will bring up a second drop down so you may select the page you'd like the customer redirected to after submission.  Selecting Show Message will bring a text box up under the drop down that allows you to create your own message to your customers after they submit their feedback form.

 

After you have made the necessary changes on the Frame Settings page, click 1save to save the changes.  This will bring you back to the Content & Layout view.

 

Form Builder

 

1.  To begin building the form, select from the left navigation the Frame that has this text attached to it: (Feedback Form).  This will expand that frame's options for settings.  Select Form Builder to begin building the form.

2. Left Label Indent, px and Right Label Indent, px refers to how far from the label the actual field is across the entire form.

3.  The Form Builder has four default rows filled out for you.  They can be altered or deleted at any time.  To edit any of the three fields that have already been created, simply mouse over the field itself and a pop up toolbar will appear.

4.  To actually edit the information in that field, click on the three lined icon.

5.  This will bring you to the Edit Control window.  You can then edit the Properties of that field and the Label, if it has one.  (Custom Text fields do not have a label.)

6.  If you do not wish to use the fields that are already there, mouse over the field and click on the "X" to delete the field.

The Edit Control Window has two parts: Properties and Labels.

 

7.  Properties allows you to set the field Title, the width, whether or not it's a required field, and Format the Title.  The title will display in Posted Data as column headers.

 

8.  Labels allows you to place the label of the field for your customer to see to the right or left, or top or bottom of the field itself.  You are able to determine the height or width of the label as well as format how it appears on the page.  Once you've made your changes, click Save.

9.  To create a new field, click on the plus sign on the far right end of the row you wish to add the field to, or to create the field on a new row.

10.  The Add Control window will appear once you click on the Edit Control button for the new field and provide you with a drop down of all the types of fields you may create.

Text Box: This is a small text field to allow customers to fill in their name or address.

Email Box: This is an email field that you can designate as the From Email in the Frame Settings for this feedback form.

Text Area: This will give your customers a large area to type in Questions/Comments.

Drop-Down List: This field allows you to create drop-downs lists with the options you want to give your customers.

Radio Group: This field only allows one choice to be made out of two or more choices.

Checkbox Group: This field allows more than one choice to be made by the customer.

Uploadable Files: This allows your customers to be able to upload a file in their feedback form if you require or ask for it.

Custom Text: This allows you to give explanation for any part of your form, or add in your business address to the top, middle, or bottom of your form.  You may type any information you need into a custom text field.

Submit Button: You may change the name of this button to anything you wish, but it will only submit the form to the email address that you selected in Frame Settings under Email To:.

Spam Submit Protection: This field uses a field that spamming software isn't able to read to be able to spam your feedback form.  It forces your visitors filling out the form to type in the characters shown in the field before submission.

 

If you are creating a new row for this field, then check the box next to Add in a new row and hit Save.  The Edit Control window will appear after you click clip0393.

 

11.  Click on the link Labels to enter the label section.  Decide where you will place the label for the field for your customers to see.  A standard width for the label is 100 pixels but be sure if you use a certain width for the initial label that you should use the same for the remaining labels and fields for uniformity.

Note:  Site templates with side navigation bars made need to use less space for their labels than site templates with top or bottom navigation bars.  You will notice a wave effect for your fields or a broken site template when previewing the feedback form page if the label widths are too great.

 

12.  Click clip0402 to save the changes to the labels and properties.

 

13.  If you are creating a large feedback form, then 1save the form often to ensure that you do not lose any data while building just as you would with a text document.

 

14.  To view the form on the page, click on Content & Layout in the left navigation, or click on Preview Page after saving the changes to the form.
